Please note that Allaymeds Compounding Pharmacy will only compound and dispense medication upon receipt of a valid patient-specific prescription from a US licensed prescriber.
For patients please have your prescriber complete and send us the form directly to us or your prescriber can contact our pharmacy at 210-809-6607 for verbal authorization.
